The following information MUST BE COMPLETED to ensure one response per lot. Incomplete surveys will be discarded.

Note: Please respond to all questions. The purpose is to fully understand the community’s preferences on both 1) should we have lighting and 2) if so, what option is preferred. If “Go Dark” is chosen, the Town of Concord will be petitioned to not install lights, however it cannot be guaranteed that lights will not be installed by the Town now or in the future.
